Our Privacy Commitments
Inclinix, respects your privacy and operates according to Safe Harbor principles. Inclinix is further committed to comply with all applicable international and governmental regulations such as the Privacy and Security Rules of the US Health Insurance Portability and Accountability Act (HIPAA), the US Department of Commerce Safe Harbor Principles, and others. Trained Inclinix officers and managers direct the implementation of effective standard operating procedures including those on the collection, use and disclosure of Personal Information (PI). PI is defined to include Safe Harbor Sensitive Personal Information and HIPAA Protected Health Information. This privacy policy applies to PI in any form including electronic or paper format.

This Privacy Policy Statement is periodically reviewed and changed as appropriate. Non-PI Collected on Your Computer Log-in
When you visit most web sites, including Inclinix operated sites, it is customary to automatically collect and temporarily store the following information about your visit:

  • Date and time of visit
  • Pages visited, and
  • Address of web site you came from to make your visit

As is also customary, we use Cookies and Internet Service Provider (ISP) information. As we use them, Cookies or ISP numbers do not personally identify you and are not PI. A Cookie is a small piece of data, which you can block or remove, that may be stored on the hard drive of your computer when you visit our website. A Cookie enables you to easily move from one interactive feature on our website to another. ISP information is a number assigned to you by an Internet Service Provider so you can access the internet. We reserve the right to use information about your visit to our website as necessary to help identify any person attempting to break into or damage our website or computers.

Personal Information Collection and Processing
Inclinix collects PI to support delivery of a broad range of medical research and consulting services in the drug, device and pharmaceutical industry. We may use PI to further inform about clinical research studies, to get comments on medical care, or to seek agreement to participate in medical research as an investigator or a patient. In some cases, we collect and process human resources data relevant to company employment activities.

In your response to outreach media such as advertisements or web sites, we may ask you to voluntarily give specific limited items of PI. When you supply PI you will have clear Notice informing you about the purposes for which the information is collected and used. At the end of this policy statement we list ways to contact Inclinix for any inquiries, corrections, updates, or disputes involving your PI. When we request permission to keep your PI for future communications, you will have the Choice to agree (opt-in) or to deny such permission (opt-out) either at the time of request or at any time thereafter. We will clearly show how to make your opt-in/opt-out choice.

Disclosure and Transfer Conditions
With your permission, we may Transfer specified parts of your PI to health care personnel at medical clinics where clinical research is conducted. Additionally, it is sometimes necessary for us to use other companies or consulting Agents to process your PI under our instructions, in performance of tasks such as sending you mail or notifying you about medical research studies. We will not knowingly disclose your PI to, or use PI from any entity that does not comply with the level of PI protection required by applicable governmental regulations. We will not disclose your PI to any non-agent third party without your permission, except as permitted or required by law. Inclinix reserves the right to transfer its database of information to another company as part of a sale or merger with the commitment that Inclinix written privacy regulations will continue to be followed and the information will only be used to fulfill the purposes for which it was originally collected.

Protection of Privacy, Security, and Integrity of Your PI
Every reasonable precaution is taken to Secure, Protect, and ensure the Integrity of your PI. We recognize our responsibility to protect your PI specifically from loss, misuse, alteration, or inaccuracy. To help ensure data integrity, we only collect PI relevant to its intended use. Our electronic information processing employs necessary and appropriate standard security measures such as user Identification/Password, Secure Socket Layer (SSL) encryption, firewalls (hardware and software barriers to unauthorized access to PI), and updated virus protection. Our computers and telephones and the information we collect on them, are kept in secure facilities within locked, environmentally controlled rooms. Our published standard procedures include reliable methods for maintaining continuity of business in the event of unavoidable circumstances.

Employees granted access to your PI are trained to manage it according to this Privacy Policy as well as current international governmental and industry standards. They are trained to safeguard its confidentiality and to ensure that it is reliable, accurate, and as current as required by the project for which it was collected.

The Inclinix Director of Information Technology is the data controller responsible for all data processing, with oversight and guidance by the company privacy officer and the internal Total Quality Management group. Your PI is maintained in confidential secure and restricted-access files for only as long as needed to fulfill the stated purpose for which it was collected.

Enforcement and Verification of Adherence of PI Activities
Inclinix operates under direction of an internal Total Quality Management group composed of the Privacy Officer and other management associates. The TQM group directs Enforcement of self-assessment activities for review and Verification of adherence to written standard operating procedures including those relevant to training and privacy practices and policies. The TQM group verifies that Inclinix Privacy Policy is accurate, comprehensive, prominently displayed, completely implemented, and accessible. Every Inclinix employee signs an employment agreement specifying sanctions up to and including discharge from employment for violation of company data confidentiality requirements.

Dispute Resolution
The Inclinix Privacy Officer will initiate investigation of complaints from people who feel the privacy of their PI has been violated. If initial resolution of the complaint is not satisfactory, Inclinix is committed to cooperating with independent entities for complaint investigation and resolution such as the EU Data Protection Authorities or the American Arbitration Association to ensure appropriate actions are taken to comply with Safe Harbor principles.
